PROPOSAL GUIDELINES

*Programs/projects must be student centered. Only proposals servicing students of the Chino Valley Unified School District will be considered.

*Proposals may request seed money or support for expanding an existing program. However, this does not guarantee funding beyond the current award and applicants are cautioned regarding the submission of proposals requiring multi-year or ongoing funding.

*CFKEF funds cannot be used to supplant available District funds. Funding will not be provided for programs or materials that are not endorsed by the District.

*Materials and equipment purchased with a CFKEF monetary award become the property of the Chino Valley Unified School District and not the grant recipient.

*Grant recipients are required to submit an evaluation report upon completion of the activity or project. Individuals or groups who do not fulfill this requirement will be ineligible for future grants.

*School/district-wide project grant applications must demonstrate how they will directly benefit students.

 

SELECTION CRITERIA

The following criteria will be used, as appropriate, in evaluating a proposal:

* Will it improve student academic achievement?

* Will it stimulate and motivate students to achieve academic excellence?

* Does it encourage specific improvement in students’ skills and attitudes?

* Does it promote a safe campus or learning environment?

* Is it conducive to a positive school climate?

* Does it include regularly defined objectives?

* Can the program/project be replicated?

* Does it support District academic performance standards?

* Is the budget realistic and does it adequately support the program/project?

* Does it reflect expenditures that directly support the proposed activities?

* Does it include a complete itemization of expenses?

* Does it require ongoing financial support and include provisions for future costs?

* Does it include an evaluation component based on the stated objectives?

 

PROPOSAL REQUIREMENTS

All proposals must be typed and follow the guidelines and format described below. Proposals also must be concise. The completed application can be submitted to John Lynde in Media Services, or mailed to: Citizens for Kids Educational Foundation, P.O. Box 328, Chino Hills, California 91709.

All proposals/applications must include the following:

1. A COVER SHEET that includes:

*Name(s) of applicant(s)

*School and/or department

*Title of project/program

*Grade level(s) served

*Number of students served

*Primary subject area: Health / Fitness, History / Social Science, Language Arts, Science / Mathematics, Special Education, Visual / Performing Arts, or Integrated

*Amount requested

2. A brief GOALS AND PURPOSE statement that identifies the purpose of the program/project and describes anticipated outcomes.

3. A PROJECT DESCRIPTION, detailed but concise, that includes any activities and/or instructional materials that are needed - or that may be developed. Indicate how the project meets standards, District goals, and/or other criteria, as appropriate.

4. An EVALUATION section, where it is explained how the success of the program or project will be measured and reported to the Foundation, as well as a description of the format of this evaluation report.

5. A BUDGET sheet that includes an itemized listing (with prices) of any materials and/or equipment that will be needed.

6. The date, and signatures of all applicants and their supervisor.
